Requests need to be submitted by the 15th of the month in order to be processed in the next payroll.
The annual maximum for combined bonuses must not exceed 20% of the employee’s total annual salary.

What is your current MPD account balance?(Required)

To calculate an accurate balance, please refer to your TNT account and click on "Cash Flow Report: Last 12 Full Months." Check if payroll has gone through for the last few months, and if not, subtract your MPD payroll amount, the 5.5% admin fee, and any MPD expenses from those months from your current balance.
